NHS Response

 

This is the response of Maidstone and Tunbridge Wells NHS Trust to the File on Four programme.

 

We are sincerely sorry that some areas of Mr Sinha’s care did not consistently meet the high standards we strive to achieve for our patients at all times.

 

We are making improvements based on the findings of our investigation, which involves team-wide learning and is not limited to any one individual or specific area of care.

 

We have sought to openly and closely involve Mr Sinha’s family in our investigation and they have helped shape the process we have followed. We thank them for their  valuable contribution and on-going involvement at this very difficult time.

 

Whilst the cause of Mr Sinha’s death is still subject to a coroner's  inquest  due  in November, we have learned lessons and made immediate changes to team handovers,  intra and post-operative haemodynamic monitoring and areas of critical care training.

 

We  have  no wish to pre-empt the work of the inquest, save to reiterate our commitment and determination to learn from every opportunity where patient care can be improved.

 

We therefore feel unable at this stage to further  comment or take part in any discussions with File on 4  or the media, other than to again offer our sincerest apologies and  regret  to the family and loved ones of our patient, Mr Sinha , and  to extend our deepest sympathy, at their tragic loss.
